Cagrilintide + Semaglutide (CagriSema)

CagriSema is a fixed-dose combination injectable of the long-acting amylin analogue cagrilintide and the GLP-1 receptor agonist semaglutide, achieving synergistic weight loss through the complementary mechanisms of "reducing hunger + enhancing satiety". Cagrilintide is a 37-amino-acid acylated amylin analogue and a non-selective agonist of amylin receptors (AMYR) and the calcitonin G protein-coupled receptor (CTR); semaglutide is a GLP-1 analogue that promotes insulin secretion and suppresses glucagon in a glucose-dependent manner. Both are white to off-white lyophilized powders with HPLC purity ≥99.0%, manufactured to CHP/USP/EP standards, for the development and CMO contract manufacturing of injectables for long-term weight management in obesity/overweight and for type 2 diabetes.

Product Overview

CagriSema is a once-weekly subcutaneous fixed-dose combination developed by Novo Nordisk, consisting of the long-acting amylin analogue cagrilintide (2.4 mg) and the GLP-1 receptor agonist semaglutide (2.4 mg). The two pathways are complementary: the amylin pathway and the GLP-1 pathway act on different satiety and feeding-regulation processes, jointly reducing hunger and increasing satiety, thereby helping patients reduce caloric intake. If approved, CagriSema would become the first combination injectable therapy of a GLP-1 receptor agonist and an amylin analogue.

Cagrilintide (development code NNC0174-0833): a 37-amino-acid long-acting acylated amylin analogue; the α-amino group of Lys1 is acylated with a C20 fatty diacid (19-carboxynonadecanoyl) via a γ-glutamate linker; a disulfide bond is formed between Cys3 and Cys8 (denoted 3→8 in INN numbering); the C-terminus is a prolinamide. Substitutions relative to human amylin are N14E, V17R, A25P, S28P, S29P, Y37P. It is a non-selective agonist of amylin receptors (AMYR) and the calcitonin receptor (CTR).

Semaglutide: a long-acting analogue built on the GLP-1(7-37) backbone, with a substitution at position 8 (Aib) to resist DPP-4 degradation, an Arg substitution at position 34, and the lysine at position 26 coupled to a C18 fatty diacid side chain via a γGlu-2×AEEA linker; albumin binding extends its half-life, supporting once-weekly dosing.

Applications

  • Long-term weight management (obesity/overweight): in REDEFINE 1 (68 weeks, 3,417 adults with obesity/overweight and comorbidities, without diabetes), assuming all patients remained on treatment, the CagriSema group achieved a mean weight reduction of 22.7% at week 68 — significantly superior to cagrilintide monotherapy (11.8%), semaglutide monotherapy (16.1%), and placebo (2.3%); 40.4% of patients achieved ≥25% weight loss. REDEFINE 2 likewise demonstrated superiority in 1,206 adults with obesity/overweight and type 2 diabetes. Novo Nordisk submitted a New Drug Application (NDA) to the FDA for the weight management indication in December 2025
  • Type 2 diabetes mellitus (T2DM): the REIMAGINE program evaluates the glucose-lowering efficacy of CagriSema; in REIMAGINE 5, CagriSema 1 mg/1 mg was non-inferior to tirzepatide 5 mg in HbA1c reduction (1.71% vs 1.67%), with superior weight loss (12.4% vs 9.1%)
  • Latest head-to-head results: in REDEFINE 4 (open-label, 84 weeks, 809 patients), announced February 2026, CagriSema 2.4/2.4 mg achieved 23.0% weight loss (20.2% under the treatment-regimen estimand) versus 25.5% (23.6%) for tirzepatide 15 mg — the primary endpoint of non-inferiority to tirzepatide was not met; the safety and tolerability profile was consistent with the GLP-1 receptor agonist class, with gastrointestinal events as the most common adverse events, mostly mild to moderate and diminishing over time
  • Mechanism and combination research: used for in vitro and in vivo pharmacodynamic evaluation of amylin receptor/calcitonin receptor agonism, GLP-1 receptor agonism, and their synergy, as well as research on feeding behavior and energy metabolism
  • CMO contract manufacturing: as a dual-peptide combination API, used in formulation development of combination injections/pre-filled injection pens, compatibility and stability studies, analytical method development and validation, and GMP scale-up and CMO/CDMO contract manufacturing

Note: CagriSema is an investigational product that has not yet been approved in the United States or the European Union; an FDA review decision is expected in 2026, and final approval status is subject to the latest official announcements. This product is an active pharmaceutical ingredient (API) intended only for legitimate industrial, research, and pharmaceutical manufacturing use; it is not for end-user clinical use or self-medication guidance.

Acotiamide Hydrochloride

Acotiamide Hydrochloride (development codes YM-443/Z-338, brand name Acofide) is the world's first orally administered gastroprokinetic drug approved for functional dyspepsia (FD), and belongs to the thiazolecarboxamide class of compounds. It is a novel, orally active, selective, reversible acetylcholinesterase (AChE) inhibitor — by inhibiting AChE it reduces acetylcholine degradation and increases acetylcholine concentration in the synaptic cleft, thereby enhancing antral and gastric body contractility, improving delayed gastric emptying, and promoting gastric accommodation. It is used for symptoms of functional dyspepsia such as postprandial fullness, upper abdominal bloating, and early satiation. This product is a white to off-white crystalline powder with purity ≥99.0%, manufactured to CHP/USP/EP standards.

Calcitriol

Calcitriol (i.e., 1α,25-dihydroxyvitamin D₃, 1,25-(OH)₂D₃, brand names Rocaltrol, Calcijex) is the **naturally active form** of vitamin D₃ produced in the body through hepatic 25-hydroxylation and renal 1α-hydroxylation, and is the most potent known vitamin D₃ metabolite. As the natural ligand of the vitamin D receptor (VDR), it acts mainly on the intestine and bone, promoting intestinal calcium and phosphate absorption, mobilizing bone calcium, and working with PTH to regulate calcium-phosphate homeostasis, while suppressing parathyroid PTH secretion. It is used for renal osteodystrophy and hypocalcemia in chronic renal failure (especially in dialysis patients), hypoparathyroidism, rickets/osteomalacia, and postmenopausal osteoporosis. This product is a white to off-white crystalline powder, **sensitive to air and light**, with purity ≥99.0%, manufactured to CHP/USP/EP standards.

Dasatinib

Dasatinib (development code BMS-354825, brand name Sprycel) is an oral, ATP-competitive second-generation dual Src/BCR-ABL tyrosine kinase inhibitor, belonging to the aminopyrimidine-thiazole carboxamide class of compounds. Its inhibitory activity against Abl and Src (IC50 approx. 0.6 and 0.8 nM, respectively) is several hundred times more potent than imatinib, and **it can bind both the active and inactive conformations of ABL kinase**, thus remaining effective against most imatinib-resistant BCR-ABL mutations (except T315I), and it can cross the blood-brain barrier. It is used for all phases of chronic myeloid leukemia (CML) in patients resistant or intolerant to prior therapy (including imatinib mesylate), and for Philadelphia chromosome-positive acute lymphoblastic leukemia (Ph+ ALL). This product is a white to off-white solid powder with purity ≥99.0%, manufactured to CHP/USP/EP standards.
